Purpose

            The Department of Psychiatry and Behavioral Sciences on the Wichita campus of the University of Kansas School Of Medicine trains psychology interns, psychiatry residents and medical students for primary health care delivery. The psychology internship program provides two intern experiences: a focused experience in neuropsychology and a newly created general clinical experience in psychotherapy and assessment.  The neuropsychology intern will have experiences in an outpatient general neuropsychology clinic, a geriatric inpatient service, a video EEG service, general medical consultation service and the opportunity to participate in Wada examinations. Some psychotherapy experience will be available to an interested neuropsychology intern through the outpatient clinic and secondary placements at other WCPIP sites.  The general clinical intern will gain more extensive therapy experience in the department’s outpatient clinic with a wide variety of psychiatric patients with an emphasis on integrating new emotional techniques based on the rapidly developing field of affective neuroscience.  This intern will also gain experience assessing personality dynamics and some intellectual and dementia screening in both the outpatient clinic and the department’s inpatient services.  Through these clinical experiences supplemented by the other educational opportunities of the internship, trainees develop the competencies that will prepare them for either a neuropsychology residency or for practice as a general clinical psychologist in a wide variety of medical settings.  In addition to providing internship training in partnership with WCPIP, the department offers a fully accredited ACGME four-year General Psychiatry Residency Program with a total of sixteen positions and a new two year Residency Program in Neuropsychology with one position. A major strength of all the educational programs is the extensive contact that the interdisciplinary faculty maintains with trainees.

The Neuropsychology Position

            Training in neuropsychology occurs in several venues under the direction of L. E. Baade Ph.D. ABPP, Cn.  As the guidelines for the practice of neuropsychology require postdoctoral training, the purpose of training is to provide an intern with preparation for a neuropsychology fellowship. We provide exposure to a combination of outpatients in the KUSM-W clinic and inpatient consultations from neurology, psychiatry, and general medicine. Patients present with varied forms of neurological insult including head injuries, vascular disease, seizure disorders, electrical shock, and toxic exposures. Some of the cases involve civil litigation and training in the detection of symptom magnification and malingering is integrated in all cases. Interns learn to attend closely to the nature and quality of test responses as well as scores. There are opportunities for behavioral observations, review of data from medical, school and work records, and corroborative interviews. An integrative approach that looks at all available data is taught along with report writing for special situations or audiences.

            The outpatient neuropsychology clinic at KUSM-W provides assessments of patients referred by general physicians, neurologists, psychiatrists, attorneys, insurance companies, and business concerns. The purpose for seeing the patient may be differential diagnosis and treatment planning or an independent neuropsychological examination for purposes of a workman’s compensation case or civil lawsuit. The presenting pathology might be as a consequence of a head injury, a neurological disease, a medical procedure or an electrical or chemical injury. Opportunities to learn and administer a wide variety of tests are provided, but generally testing of outpatients is provided by neuropsychological technicians. The focus for the intern is more on the interpretation of test and other data and report writing. Report writing is tailored to the nature of the referral questions and the
audience to which it is directed.

            A close relationship has been developed between the department of Psychiatry and Behavioral Sciences and the regional epilepsy center at Via Christi Regional Medical Center. Neuropsychology and psychiatry consultations are written for the majority of epilepsy center patients admitted to the hospital for Video EEG monitoring. This unit provides an opportunity to be involved in the differential diagnosis of patients with epileptic and non-epileptic seizure disorders. Some of the patients on the VEEG unit are candidates for lobectomy or for experimental implants to control seizures. These patients are seen on an outpatient basis for more complete neurocognitive evaluations, and then again in the hospital setting for Wada examinations. Occasionally patients are candidates for functional mapping during surgical procedures.

            In the area of geriatric neuropsychology, training is provided in collaboration with Connie Marsh MD, a KUSM-W faculty member and the director of the Senior Behavioral Health unit at the St. Joseph campus of the Via Christi Medical Center. Dr. Marsh and other physicians request neuropsychological consultations on geriatric patients hospitalized on the unit, and on the medical and skilled nursing units of the Via Christi hospital system. Additionally, Dr. Baade is the consulting neuropsychologist for the geriatric psychiatry unit at the Susan B. Allen Memorial Hospital. This is the general medical hospital in the rural town of El Dorado in Butler County, Kansas. Dr. Baade also maintains an active outpatient geriatric service. Both inpatient and outpatient cases may involve forensic issues of competency and the need for a guardian and/or a conservator. Interns, along with psychiatry residents and medical students participate in a consultation seminar with Dr. Baade, Dr. Marsh and Dr. D’Empaire, the KUSM-W consultation and liaison psychiatrist. The goal of this educational service is to develop professional psychology skills to work with the growing population of geriatric patients. These skills include neuropsychological assessment of the elderly and patient and family education regarding delirium, dementia, and health care compliance issues.

Research

            While the primary focus of the internship program is clinical neuropsychological assessment, opportunities for participation in research are provided. Interns that plan to apply for a neuropsychology residency (postdoctoral fellowship) are encouraged to rapidly complete their dissertation if that has not been accomplished. They are also encouraged to participate in ongoing research projects with the goal of a poster at a national convention and/or a publication submission during the internship year.

            An interested neuropsychological intern could have some opportunities for some therapy and personality assessment experience through work with Drs. Veenstra and Morgan as described in the next section.

General Clinical Therapy and Assessment Position

            Providing therapy and conducting assessments will be the key features of this internship experience.  The newly established position will provide these services for an expanding psychiatric outpatient clinic.  Clients will include adults, adolescents, and the elderly who have psychiatric problems such as anxiety and affective disorders, medical problems, and emotional and relational problems.  Work with older children, couples, and families may be possible.  Establishing group therapy for select populations like anxiety or affective disorders is another possibility. The main therapeutic approaches utilized in the clinic are supportive, cognitive-behavioral, interpersonal, and dynamically guided brief therapies, but family systems and biological functioning are integrated into these approaches.

            The assessment experiences in the outpatient clinic will involve personality testing, cognitive testing and health psychology assessments. The most common instruments used will be the MMPI-2, MMPI-A, MCMI-III, MACI, and MBMD.  While personality testing will be common, some assessments of intellectual functioning and health functioning are completed when requested.  There will be opportunities to do assessments at the Good Shepherd inpatient psychiatric hospital where an occasional dementia or IQ screening will be added to the mix of instruments used. The intern will gain experience integrating test results, conducting diagnostic interviews and writing reports.  The intern will also gain experience providing verbal feedback to medical students, residents and attending psychiatrists.

            Assessment supervision will be provided by Dr. Morgan who is an expert in the interpretation of personality dynamics using instruments such as the MMPI-2, MMPI-A, MCMI-III and MACI.  He has done research in this area in cooperation with the psychology program at Wichita State University, and the intern will have the opportunity to participate in these research experiences if he or she desires it.  There may also be opportunities to set up and participate in clinical research protocols that will be developed as the outpatient clinic expands its services.  Though Dr. Glenn Veenstra serves as the coordinator of therapy in the outpatient clinic, both he and Dr. Don Morgan supervise the interns on their psychotherapy cases using both scheduled supervision times and conjoint involvement in on going case discussions. Dr. Morgan provides supervision in practical approaches to therapy using a variety of techniques. He is especially interested in the use of humor in therapy and has special training in the treatment of sexual dysfunction.

            Under the supervision of Dr. Glenn Veenstra interns will learn to integrate two important additional frameworks into their basic therapeutic skills. The first is a deeper understanding of human emotional functioning that grows out of recent neuroscience research. An appreciation of the importance of emotions corrects the narrowness of the cognitive perspective and provides a powerful way to explain symptoms to patients. It also provides new techniques that increase the effectiveness of cognitive interventions.

            The FULFIRS model developed by Dr. Veenstra provides the second unique addition to the outpatient therapy experience. It integrates interpersonal functioning in family and relationship systems with individual personality dynamics. The model provides guidelines for assessing the influence of the social system on a patient's presenting problems. This leads to additional methods of intervening by adding dysfunctional relationship thinking to the cognitive corrections and by adding ways of involving the spouse and other family members in the treatment of an individual's presenting problems. Dr. Veenstra is an AAMFT approved clinical supervisor so work with him can count towards membership in this organization.

General Information about the Internship

            Active and supportive supervisory relationships are developed where interns are encouraged to understand and strengthen their own professional and personal abilities. Professional development is facilitated by participation in educational programs, case conferences, and community seminars. Interns may participate in faculty research programs and teaching/consultation opportunities if they desire.

            The stipend at KUSM-W is projected to be $18,643 per year (pending budget approval). Individual health insurance is provided, but there is a three-month waiting period before it takes effect. FICA and income tax will be withheld.
