Appointments

We are currently accepting new patients..
All visits are handled by appointment and scheduled with the physician of your choice whenever possible. Call (316) 293-2622 and follow the voice prompts to reach our scheduling department.

If you are a new patient, you will receive a packet of information with the necessary forms and information. Please check in at the reception desk 20 minutes prior to your scheduled appointment time with your completed forms. The receptionist will gather the information from you, along with your insurance information. We will be happy to file your incurred charges with your insurance company on your behalf.

No Show Appointment Policy

If you are unable to keep your appointment, we require 24 hours notice of cancellation. To ensure availability of appointments to our patients, we have adopted a “no show appointment” policy. If you miss three appointments, your physician may review your records to determine if you'll continue to be seen at our clinic.

Referrals

If you have insurance that requires a referral, please call your primary care physician to have a referral mailed to us or faxed to (316) 293-1866. The referral must be received prior to the scheduled appointment. Any charges incurred that are not covered by your insurance company due to lack of a referral will be your responsibility.

We require 48 hours advance notice for completion of a referral for visits to other physician offices and to emergency rooms. Same day referrals will not be done except for emergencies. Our staff will ensure that you are going to the right physician for your insurance plan.

Financial Responsibility

You are responsible for all charges incurred for your medical care.

Non-Covered Services

You are responsible for services that are denied by Medicaid or other insurance as non-covered services.

Minimum Payment of $50

If you do not have insurance, you will be required to pay a minimum of $50 at check in towards the charges you incur. You must make payment arrangements for the remaining charges.

Co-Pay

If you have insurance that requires a co-pay, the co-pay must be paid at check in.
If you cannot pay the co-pay, your appointment will be rescheduled.

Prescription Refills

Call your pharmacy for refills. The pharmacy will fax us the request. Prescription refill authorizations will be processed Monday through Friday, 8:00 a.m. to 5:00 p.m. ONLY. If a written prescription is required, the nurse will call when it is ready for pick up. Allow 48-hour turnaround to get prescriptions refilled.

Hospitals

As a group, we utilize area hospitals. Our primary hospitals are Wesley Medical Center and Via Christi-St. Francis, where our hospitalist teams care for our hospitalized patients. Your hospital admission will be determined by your personal preference and/or the requirements of your health insurance company.
