Facilities Forms - Parking Violation Appeal Form

A violation must be paid or appealed to Parking Services within fourteen days from the date on which the violation was isssued. The appellant must complete either this on-line form or a paper form obtainable at the Facilities Management Office in B050, KUSM-Wichita.

Late Appeals (Appeals initiated after fourteen days from the date of the ticket) must be accompanied by the payment of the fines for the violation(s) to be appealed. Late Appeals can not, therefore, be done on this online form. All rights of appeal are forfeited if no appeal has been made within 90 calendar days from the date on which the ticket was issued.

Please note that only one ticket can be entered at a time on the online form. Send a separate form for each additional ticket you wish to appeal.
