Educational Technology offers bubble sheet scanning
(technically known as Optical Mark Recognition [OMR] scanning),
which includes:
- Survey tabulation
- Course or instructor evaluation tabulation
You
can use either blue or black ink or pencil on your form. You
receive either the raw data from the scan, or Educational
Technology can import the data into Excel.
You can also purchase multi-purpose scan forms sheets
(bubble-sheets) from Educational Technology for your projects.

Scanning Process
NOTE: Contact Educational Technology to discuss your
project before ordering any scan forms or purchasing forms from
Educational Technology. There are several scan form requirements in
order to use the scanner.
- Step 1: Once you have completed your data collection, bring
your scan sheets and 3-4 blank scan sheets to Educational
Technology.
- Step 2: An application will be created, and your sheets will be scanned and a data file generated.
If you requested your data be imported in Excel, the data
will be converted and formatted.
- Step 3: The data file will be sent via e-mail to you. You can
pick up your scanned sheets after you receive your data
file.

Asked Questions
Can people fill out my survey in pen?
Pencil is the preferred marking tool, however, blue or black ink can be used. Anything else will not be read by the scanner.
If I need a custom form designed, whom do I
contact?
Contact Educational Technology and we will put you in contact
with NCS-Pearson
and they will assist you designing and printing a custom
form.
What if I only want a couple of forms scanned? Does it cost
me more to do a small quantity?
No.
We can scan one form, or we can scan thousands at the same price
per sheet.
Do you have forms that I can purchase?
Yes.
We have multi-purpose scan sheets, ranging from 40 answer to 100 answer that should meet most needs you
