Conflict of Interest (COI) Form:
The University of Kansas School of Medicine – Wichita requires annual Conflict of Interest reporting per Kansas Board of Regents’ policy. This reporting requirement must be completed by August 31 each year.
Annual COI reporting is required of:
- ALL KUSM-W faculty and unclassified staff
- ALL employees, students or residents participating in KUSM-W research
Principal Investigators (PIs) must ensure that all research personnel, staff, students, and residents participating in their research complete this form.
Statement of Substantial Interests (SSI) Form:
While questions are very similar to the COI form, the SSI form is required annually in April for faculty members who receive an annual salary of $50,000 or more; and other identified employees who hold a major policy making position. This form is required by the State of Kansas (vs the Board of Regents) and mailed to the employee’s home address.
Failure to complete the SSI Form is a class B Misdemeanor and can result in a fine.
