Compliance Forms and Instructions

If your research study involve data or specimens that already exist in their entirety at the time of IRB submission please complete the RETROSPECTIVE APPLICATION.

If your research study involve data or specimens that have not been created or collected yet please complete the PROSPECTIVE APPLICATION.

NOTE:  If your research study requires a full committee review or does not “fit” within the above applications, you must complete the respective KUSM-W, WMREF or VCRMC full committee forms. KUSM-W forms can be found on the KUSM-W forms pages.
